Titanium contains no nickel as an alloying element, and a nostril is a worse environment than an earlobe: warmer, damper, slower to heal, and knocked by tissues, towels and glasses every day.

Is the metal named, with a grade? "Titanium" is a material claim. "F136" or "G23" is a material claim pointing at a published document. The second is worth more.

Why a nostril is harder than a lobe
An earlobe is soft tissue with a good blood supply, and a healed lobe piercing is lined with skin. A nostril is neither of those things. It sits at the edge of cartilage, it heals over four to six months rather than six weeks, and the inside of it is permanently moist.

Gauge: the thing to settle first
Twenty gauge is roughly 0.8mm and is the common nostril size in the United States. Eighteen gauge is roughly 1.0mm and turns up on needle piercings and on work done outside the US. A 20G post in an 18G channel moves around, which abrades the inside of the piercing and produces irritation that looks exactly like a metal reaction and is not one.
