A second lobe piercing is not simply another first one. It sits higher on the lobe where the tissue is thinner, it is usually spaced six to eight millimeters from the hole below, and whatever you put in it has to clear that lower earring. So the shopping question changes: not just “is the metal stated” but “is it small enough, flat enough and light enough”. Slim huggies and flat-back studs in stated titanium answer all four.
How we judged these
Criteria first, ranking second. Nobody here has worn these earrings; the scores rate what each seller discloses and how the named metal suits reactive skin. What we weighted, in order:
Is the alloy stated, and how specifically? “G23” or “ASTM F136” beats “titanium”, which beats “hypoallergenic” with no metal at all.
Does the fitting create a second component? A hinged huggie and a flat back do not. A butterfly clip does, and it is the most under-disclosed part in this whole category.
Clearance. Size and profile, because an earring that sits on its neighbor is a problem no metal fixes.
Cost per hole. Filling three holes properly should not cost three times filling one, and sets exist that do it honestly.

Why the second hole misbehaves more than the first
Three things change as you move up the lobe. The tissue gets thinner and has less blood supply, so it heals more slowly. Those spacings are the common range rather than a rule — every ear is different and your piercer placed yours where your lobe allowed. Measure your own before buying hoops: the gap between holes, in millimeters, is the largest hoop diameter that will hang clear.
The back problem, doubled
A butterfly clip is a separately manufactured part, frequently made of a cheaper alloy than the earring it holds, and almost never described in the listing. On a single piercing that is a nuisance. On a stacked ear it is a nuisance twice over, and the two clips sit within a few millimeters of each other behind a lobe you then sleep on.
A flat back — a smooth disc that sits flush — removes the clip entirely, and a hinged huggie closes on itself so there is no second component at all. Both are on this page for that reason.
